• Maintains record availability by processing delivery and retrieval using chart mark-off procedures; and facilitating chart location activities.
• Retrieves medical records by following chart-out procedures; documenting reasons charts cannot be retrieved for statistical and follow-up purposes.
• Delivers charts to assigned areas of the hospital by following established routing procedures.
• Keeps health care providers informed by communicating the availability or unavailability of the record.
• Collection of Emergency Department records as per the schedule.
• Delivery of health records in the approved delivery system while ensuring safety confidentiality and security.
• Sort loose information in the appropriate format and sorter.
• Filing of charts in the appropriate storage /filing areas.
• Interdepartmental shredding as per identified documents.
• Match all loose reports using at least two patient identifiers.
• Secure work area.
• Collection, as per the schedule at pre-defined periods.
• Maintains confidentiality of all data and information at all times.
• Delivery of requested records as per the need and the request.
• Prints pull list and tracer cards of booked clinic appointments prints clinic add-on list and prepares patient records accordingly.
• Appropriate usage of chart tracking module ensuring that patient record locations are continuously updated.

High School Diploma preferred.
